The Shock Qualification by Analysis Acceptance Form provides identification of the results of equipment shock qualification by analysis, status of qualification, method of qualification, and a record of the qualification. The form is used to record items shock qualified in accordance with the requirements of T9070-AJ-DPC-120/3010 and PPD 802-6337447.
a. This data item description (DID) contains the format and content preparation instructions for the Shock Qualification by Analysis Acceptance Form resulting from the work task described by section 7.3 of T9070-AJ-DPC-120/3010.
Preparation Instructions
1Reference Documents.The applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as specified in the contract.

2Format.The Shock Qualification by Analysis Acceptance Form shall be in the format specified in NAVSEA 9072/XX.
3Content.The following information shall be included in the Shock Qualification by Analysis Acceptance Form (line items below refer to Shock Qualification by Analysis Acceptance Form line items):
3.1Qualification Method (line item 1).The method used to analyze the item for qualification (i.e., qualification by DDAM, static g calculation, different transient approaches, or extension). Analysis extension requirements (if permitted) are provided in ship specifications.
3.2Item (Description) (line item 2).This section specifically describes the item analyzed.
3.3Part Number (line item 3).This section specifies the Manufacturer or Integrator and Manufacturer Part Number (MPN) of the item analyzed.
3.4Manufacturer (line item 4).This section specifies the Manufacturer or Integrator of the item analyzed.
3.5Ship Class (line item 5).This section identifies the ship, ships, or ship classes for which this equipment was shock qualified.
3.6Model Number (line item 6).This section describes the specific model of the item analyzed, including all applicable information regarding model numbers, serial number, type, etc.
3.7Contract Number (line item 7).This section identifies the U.S. Government contract number under which this equipment was procured and shock qualified.
3.8Size/Capacity (line item 8).This section identifies the size, weight, and other physical description of the component.
3.9Controlling Drawing Number (line item 9).This section identifies the manufacturer's drawing number for the item analyzed. If the item is covered by more than one drawing number, either the top-level drawing number, a set of drawing numbers, or both shall be referenced. The actual revision letter and date of the drawing revision to which the equipment was assembled shall be referenced.
3.10Controlling Military Specification (line item 10).This section identifies the military specification (if any) that the item was manufactured too. The actual revision letter and date of the military specification shall be referenced.
3.11Service (line item 11).This section identifies the intended service of the item to be qualified (e.g., Air [HP/LP], Communications, Electrical, Electronics, Hydraulic, Machinery, Other Fluid Systems, Outfitting, Ventilation, Water [Fresh, Potable, Chilled], Water [Sea]).
3.12Agency/Contractor Completing Shock Analysis (line item 12).This section identifies the agency or contractor who performed the shock qualification analysis.
3.13Reference to Analysis Plan/Approval for Qualification by Analysis (line item 12a).This section identifies the reference for approval of shock qualification by analysis from the program or technical authority.
3.14Analysis Report Number, Revision, and Date (line item 13).This section identifies references (and dates) relevant to the qualification by analysis. For equipment shock qualified by analysis, references to the mathematical model report and dynamic analysis report shall be included. In addition, reference to any supplemental reports shall also be identified.
3.15Previous Shock Analysis or Test Approval Reference (If this form conveys shock extension approval) (line item 14).This section identifies the reference to the basis for approval by shock extension. Revisions and dates of reference shall be documented.
3.16Shock Grade (line item 16).The shock grade of the equipment, as defined by MIL-DTL-901, shall be identified.
3.17Equipment Type (line item 17).The type of the equipment, as defined by MIL-DTL-901, shall be identified.
3.18Equipment Class (line item 18).The class of the equipment, as defined by MIL-DTL-901, shall be identified.
3.19Mounting Location, including Ship Type (line item 20).The mounting location, as defined by T9070-AJ-DPC-120/3010 for shock qualified equipment, shall be identified.
3.20Mounting Plane Aboard Ship (line item 21).The shipboard mounting plane of the item represented during shock analysis, as defined by MIL-DTL-901 for shock qualified equipment, shall be identified.
3.21Mounting Orientation of Item (line item 22).The shipboard mounting orientation of the item, as defined by MIL-DTL-901 for shock qualified equipment, shall be identified.
3.22Remarks/Approval Limitations (line item 23).This section includes any pertinent remarks or limitations of the shock qualification.
3.23Approvals (line item 24).This section consists of handwritten or electronic signature(s) indicating Technical Authority approval that ship equipment meets shock qualification technical requirements and the user's functional or operational requirements are satisfied.
4Documentation Requirements.Documentation requirements shall be as specified in the contract or order.
