The Installation Deficiency Test Report provides documented data and results of the testing conducted in accordance with the Commissioning (Cx) Plan and Installation Test Procedures work task described in the contract. The Installation Deficiency Test Report provides an analysis of the data and results to identify which specifications were or were not attained. After facility acceptance, the data shall be used for reference by the operations and maintenance personnel.
This Data Item Description (DID) contains the format, content, and intended use information for the Installation Deficiency Test Report resulting from the work task described in the contract. It is intended that all requirements contained herein shall apply to all applicable Department of Defense acquisition programs through all program phases.
Preparation Instructions
1Reference Documentation.The applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as specified in the contract. List the applicable reference documentation.
2Format:The Installation Deficiency Test Report shall be in contractor format. Format shall be machine-readable by the Government.
3Content:The Installation Deficiency Test Report shall contain the following:
3.1Cover Page and Document Tracking Information:
3.1.1Cover Page:Title, Date of Issue, Revision Date, Contract Number, Contractor's name and address, Distribution Statement (ref: DoD Instruction (DoDI) 5230.24 (https://www.esd.whs.mil/Portals/54/Documents/DD/issuances/dodi/523024p.pdf)), Export Control Warning Label, and Security classification.
3.1.2Record of Change Page:A record of all changes made to the Installation Deficiency Test Report.
3.2Table of Contents:A listing of the major sections identified below in 3.3-3.9.
3.3Overview:A description of the purpose of the Installation Deficiency Test Report and the system(s) to which it applies.
3.4Test Description:Provide a description of the test.
3.5Data Sheets:The data sheets shall contain the actual measurements (in English and metric or as specified by the contract or in writing by the procuring contract officer) made on each facility and equipment during the pre-shakedown, shakedown, and operational tests, and the acceptance limits or criteria for each measurement.
3.6Test Equipment:A list of all test equipment used for the tests, including manufacturer and model number, the test date, and the name of the activity performing the calibration of each test equipment item.
3.7Item or Parts:A list of items or parts used during the installation testing. Denote if the item is a Configuration Item, mission-critical item, or safety-critical item (ref. 3.8Major Equipment and Facilities:Provide a list of major (as defined in the contract or in writing by the Procuring Contract Officer) equipment/facilities installed and tested at each location.
3.9Analysis:An analysis of the test conducted and a description of the results documenting deficiencies and issues. Include a description of how deficiencies and issues were identified, evaluated, and analyzed. Provide a risk analysis based on the deficiencies and issues discovered during the test. Include a description of correction and re-testing requirements and activities to provide deficiency and issue resolution.
