This Data Item Description (DID) contains format and content preparation instructions for the data product generated by the specific and discrete task requirement as delineated in the contract.

The Maintenance Service Report will document repairs made to each piece of equipment.

Preparation Instructions

1Reference documents.The applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as cited in the current issue of the DODISS at the time of solicitation; or for non DODISS -listed documents, as stated herein.

2Format.The report shall be in contractor's format.

3Contents.The report shall contain the following:

3.1Equipment information list.This list shall include:

3.1.1Date received.

3.1.2Transaction number.

3.1.3Decal number.

3.1.4Equipment.

3.1.5Manufacturers code.

3.1.6Model number.

3.1.7Serial number.

3.1.8Contact name.

3.1.9Organization.

3.1.10Building.

3.1.11Phone extension.

3.2Reason for submission.

3.3Symptoms.

3.4Work performed on equipment.

3.5Parts.The service report shall include a list of parts used with the following information:

3.5.1Circuit symbol.

3.5.2Description.

3.5.3Part number.

3.5.4Quantity.

3.5.5Price.

3.5.6Amount.

3.6Time and Charges.A record should be kept of the time and charges for each of the following: Calibration Lab. Contractor. Job Order Total.

3.6.1Time.The time shall include the following:

3.6.1.1Calibration.

3.6.1.2Repair.

3.6.1.3Total.

3.6.2Charges.The charges shall include the following:

3.6.2.1Labor.

3.6.2.2Parts.

3.6.2.3Other.

3.6.2.4Total.

3.7The contractor shall specify a single point of contact in order that services may be ordered.
