Description & Purpose
This report is designed to detail all relevant facts concerning any oil discharge from the vessel(s) being constructed hereunder, whether or not caused by the contractor's negligence, or by (1) an act of God, (2) act of war (3) act of negligence on the part of the Government, or (4) an act of a third party whether or not such act was not negligent, or any other combination of these causes.
Application & Interrelationship
7.1 This Data Item Description (DID) contains the content and format preparation instructions for the data product generated by the specific and discrete task requirement as delineated in the contract.
7.2 A copy of the reference document cited below may be obtained as specified in the contract.
7.3 This DID supersedes UD1-A-23041.
Preparation Instructions
10.1Reference DocumentThe applicable issue of the document cited herein, including its approval date and dates of any applicable revisions and changes, shall be as specified in the contract.
10.2GeneralAlthough it is desired that this report be concise, sufficient detail is desired. Vague descriptions and indications of "unknown" are not acceptable unless the Contractor can show to the satisfaction of the Supervisor that such descriptions and indications are not susceptible to more detailed definition. Except where impossible, the data in this report shall be from the result of an "on-site" review.
10.3FormatThe Oil Discharge Report shall be prepared in contractor's format.
10.4ContentThe report shall be as specified in OPNAVINST 5090.1 Appendix entitled "Oil Spill Message Format" and shall also include the following:
10.4.1Type of discharge (at sea, in port, ashore).
10.4.2Date, time, and location of discharge(for at-sea discharge, specify latitude and longitude at the beginning and end of the incident).
10.4.3Type of ship, vessel, craft, or facility involved.
10.4.4Nature and quantity of oil discharged("oil" is defined to include gasoline and jet fuels).
10.4.5Weather conditions at time of discharge(wind speed and sea state).
10.4.6Specific cause initiating discharge.For example:
10.4.6.1Major and Minor collisionswith identification of object collided with
10.4.6.3Difficulties with at-sea fueling operations
10.4.6.4Mechanical and structural failures
10.4.6.5Human error(give details except personnel identification not desired)
10.4.6.6Mechanical/design deficiencies
10.4.6.7Oil in bilges(give reason(s) for contamination)
10.4.6.8Handling of oily wastes in donuts
10.4.7Corrective measures recommended to prevent a future discharge of this nature(except) discharges required for operational reason(s).
10.4.8Any other comments considered germane.
