This report will be used to present the information concerning grounding, damage and collision of a ship or boat. This report is required for ships or boat damage which are covered and not covered by insurance.

This Data Item Description (DID) contains the format and contents preparations for the data product generated by the specific and discrete task requirement as delineated in the contract.

10.1Format.The report shall be presented in the contractor's own format.

10.2Media Requirements.The media shall be as specified in the contract.

10.3Content.The report shall contain the facts of the incident, including:

10.3.1Date and time.

10.3.2Flood state.

10.3.3Latitude and longitude.

10.3.4Sea water.

10.3.5Weather Conditions.

10.3.6Ships and vessels in assistance.

10.3.7Date time group of related messages.

10.3.8Witnesses.

10.3.9Diver's report.

10.3.10Difficulties experienced in getting off shoal water.

10.3.11Reasons for grounding.

10.3.12Extenuating circumstances, as applicable.
