The Significant Maintenance Historical Documentation report is required to provide serialized tracking of end items and components repaired during overhaul and will be used by Equipment Specialists, Item Managers, and Engineers to determine the reliability and life span of a part.

a. This Data Item Description (DID) contains the format, content, and intended use information for the data deliverable resulting from the work task described in the solicitation.

b. This DID supersedes DI-MGMT-81564.

Preparation Instructions

1Reference documents.The applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as specified in the contract.

2Format.AFTO Form 95, Significant Historical Data, or contractor generated facsimile shall be used. (Copies of this form are available online at http://www.e-publishing.af.mil/.)

3Content.Significant maintenance actions during overhaul/repair shall be entered in accordance with the Maintenance Historical Documentation chapter found in Technical Order (TO) 00-20-1, Aerospace Equipment Maintenance Inspection, Documentation, Policies, and Procedures.
