The Condition Found Report (CFR) will be used by the Government to identify needed repairs and recommend corrective action for work discovered during the contract performance period, which are not covered by the work specifications. The CFR specifies if the issue has a technical, cost, or schedule impact and recommends corrective action.
This DID contains the format, content, and intended use information for the data product resulting from the work task described in the contract.
This DID supersedes DI-MGMT-81648.
Preparation Instructions
2Format.The CFR shall be in a format similar to Sample Figure 1 below. If there is a cost impact to the CFR, cost details shall be provided in a separate accompanying document; contractor format shall be acceptable for the cost impact.
3Content.The report shall be serialized by CFR Number, and shall include the following:
3.1Title of CFR, project name, contract number, task order number, ship name, and hull number
3.2Work Item Number to which the CFR relates
3.3Specific location of the work requirement
3.4Date work requirement was discovered, date CFR was submitted, requested response date, period of performance end date of the contract
3.5Impact type (Technical, Schedule, Cost, or any combination of all three) and clear description of the impact
3.7Recommendation if a Liaison Action Request (LAR) or Period of Performance (PoP) extension is required to resolve the issue
3.8Clear description of the work requirement
3.9Recommendation for corrective action
3.10Recommendation for appropriate or best time to accomplish the corrective action(i.e. during current availability with or without schedule change, future CNO or Continuous Maintenance Availability), and supporting rationale for the recommendation (such as: cost efficiencies, availability of work force, availability of material, premium expenditures, etc.).
3.11Cost and time estimates and the time frame for which it is valid
3.11.1Class "C" cost estimates.For work requirements that could be estimated within five working days, a class "F" estimate (+/-40%) identifying any potential impact to the current schedule shall be included. The Class "F" estimate shall also contain the date on which a class "C" estimate is provided.
3.11.2Estimated Premium/Acceleration Costsincluding premium costs for material, subcontractors, man-hours, rework, and any additional costs to ongoing work resulting from inclusion of the CFR work requirement.
3.11.3Identification of related changesif any, to the internal milestones and production and contract completion dates shall be included. When no changes were required a statement to that fact shall also be included.
3.12If the CFR has a cost impact, detailed financial information shall accompany the CFR.
3.12.1CFR serial number which the cost impact document is accompanying.
3.12.2Financial details of the contract broken out by Contract Line Item Number (CLIN)to include: ceiling awarded for each CLIN, funding obligated on each CLIN to date, and estimated weekly expenditures (burn curve) per CLIN.
3.12.3Detailed breakdown of the estimated costs.For cost estimates that include labor expenses, this breakdown shall include description of labor category, quantity of hours per labor category, hourly cost per labor category, total cost per labor category, and total labor cost (including any added fees). For cost estimates that include Other Documented Cost (ODC) expenses, this breakdown shall include a description of each expense, unit of measurement of each expense, cost per unit, quantity of units, total cost per expense, and total ODC cost (including any added fees).
4Media.The electronic media requirements shall be as specified on the DD 1423 CDRL.
