The report documenting the investigation results of a critical defect identifies the occurrence(s) of critical defects and their root cause, segregated suspect material, planned corrective action, and the request for authority to restart.

This Data Item Description (DID) contains the format and content preparation instructions for the data product generated by the specific and discrete task requirements as delineated in the contract.

Preparation Instructions

1Format.The Safety Report shall be in contractor format.

2Content.The report shall include a narrative description of the detailed investigation and analysis of the critical defect and shall include:

2.1The root cause of the defect.

2.2Evidence that the segregated material is not affected.

2.3Corrective action(s) taken to eliminate the defect in remaining material and to preclude recurrence.

2.4Any detailed diagrams, drawings, sketches, photographs, etc. needed to provide clarification on identification of items, parts, assemblies, and materials.

2.5Segregated Material Use and Restart Request:This section shall include:

2.5.1Reference to the approved notification, detail investigation and corrective action(s).

2.5.2Identity of the material, lot number, and quantity.

2.5.3Narrative requesting "use" or "restart" authority.

3Media requirements.The report shall be in Adobe electronic Portable Document Format (pdf), version 2007-2010.
