This DID will identify the required information needed to obtain authorization to deviate from the references of any work item.

This DID contains the format content preparation instructions for the data product generated by the specific and discrete task requirement as delineated in the contract.

Preparation Instructions

1Format.The report shall be in the contractor's format.

2Content.The report shall contain the following information:

2.1Ship's Name/Hull Number

2.2Contractor/Subcontractor

2.3Name of deviation requestor

2.4Identification as an existing condition and result of repair

2.5Duration of repair with and without the deviation

2.6System, component as indicated in the applicable selected record drawing

2.7Location by compartment, frame and deck

2.8Description of the deviation and degree of non-compliance

2.9Document deviated from

2.10Technical justification for the deviation

2.11Alteration number if applicable

2.12Date supervisor response is needed by
