This DID contains detailed information about specific items containing PCBs. It is used for the control of eventual disposal of equipment containing PCBs.

This DID contains the format and content preparation instructions for the data product generated by the discrete task requirement as specified in 40 CFR 761 delineated in the contract.

Preparation Instructions

1Reference documents.The applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revision, shall be specified in contract.

2Format.The report shall be in the contractor's format.

3Content.The report shall contain all of the information specified in standard 40 CFR 761 and shall also include the following: Installation name EPA identification number Office and phone number of the person preparing the inventory Date prepared using the following format: yy/mm/dd The report shall include In-service PCB items identifying the following: Serial/Id # - The identification or serial number found on the PCB containing item. Ship/Compartment No. - Location of the PCB containing item. PPM/Date - The amount of PCB contained in the PCB items and PCB contaminated items as tested, expressed in parts per million (ppm). If the item is sealed or has not been tested, the information be specified on the label. The Report shall include the date the analysis was performed, if applicable and shall identify the following: Equipment Type - Type of system and equipment PCB items is in. Manufacturer - Information from equipment label, if available.
