The Operation and Maintenance Report will identify the Operation and Maintenance information needed for blasting operations, to ensure that the Contractor is operating in compliance with environmental law.

This DID contains the format and content preparation instructions for the data product generated by the specific requirement delineated in the contract.

Preparation Instructions

1Format.The Reports shall be in contractor's format.

2Content.The report shall contain the following information:

2.2CDRL exhibit number

2.3Approval/submission

2.4Project name

2.5Contract number

2.6Contractor's name and address

2.7Location

2.8Equipment serial number, manufacturer and Model number

2.9Date, time and shift

2.10Printed and typed name of operator/inspector

2.11Operating parameters and acceptance criteria.

2.12Problem, Condition

2.13Action taken, date and time of correction
