This DID will be used by the Naval Supervising Activity (NSA) to verify submission of material used to perform contractor maintenance into the 3-M/OARS database.

This DID contains the format and content preparation instructions for the data product generated by the specific and discrete task requirement as delineated in the contract.

Reference cited in the DID can be obtained from the Naval Sea Logistics Center, 5450 Carlisle Pike, P.O. Box 2060, Building #307, Mechanicsburg, PA 17055-0795.

Preparation Instructions

1Reference Documents.The applicable issue of documents cited herein, including approval dates of any applicable amendments, notices and revision shall be as cited in the contract.
2Format.The Reports shall contain all material used in the performance of contractor ship maintenance and shall be in a format similar to that of Figure 1.
3The Report shall include a title pageidentifying the following:
3.1Title
3.2Total number of maintenance material transactions submitted by month.
3.3Summary of any submission issues.
3.4Imbedded charts/embedded graphs showing the maintenance material monthly transactions for the appropriate quarter.
3.5Preparing activity or contractor's title and telephone number.
