The Contractor Performance Assessment Report will provide the Government with the Contractor's assessment of their performance.
This data item description (DID) contains format and content preparation instructions for the data product generated by the specific and discrete task requirement as delineated in the contract.
Preparation Instructions
1Format.The Contractor Performance Assessment Report shall be in the Contractor's format.
2Content.The Report shall contain the following Sections:
2.1Training Requirements/Qualifications.This section shall identify all mandatory training requirements/qualifications in support of the contracts requirements. It shall also contain status of mandatory training/certificates in percentage of total completion and show status on upcoming qualifications accompanied and how all documentation for the designated reporting period was verified.
2.2Timeliness of filling personnel requisitions.This section shall identify the average number of days required to fill each requisition during the reporting period in comparison to industry average. It shall also identify what efforts are being taken during the hiring process to fill challenging personnel requisitions.
2.3CDRL Quality and Timeliness.This section shall contain a list of all required CDRLs along with their respective delivery frequency, metrics as to the timeliness, and accuracy of submitted CDRLs for the given reporting period. This section shall also identify the overall accuracy rate objective, achieved CDRL accuracy results and overall rate of error(s) requiring CDRL revision.
2.4Occupational Near-Mishap/Injury/Illness Reporting.This section information concerning occupational safety incidents occurring in support of contract requirements and shall include each incident, date of incident and the percentage of incident reports submitted within eight (8) working hours of the incident, and the overall injury rate for the contract period in comparison to the industry standards.
2.5Personnel Retention Rates.This section shall contain personnel retention rates both overall and categorized and identified with reason for separation.
2.6Media Requirements.The Contractors Performance Assessment Report shall be in Microsoft Power Point, version 2007-2010.
