The Certificate of Compliance provides certification from the contractor that the materiel supplied is in full compliance with all contract requirements. This DID contains the format, content, and intended use information for the data deliverable required by the contract.

This DID supersedes DI-MISC-81356.

Preparation Instructions

1Format.The Certificate of Compliance shall be presented in a format similar to that of Figure 1.

2Content.The Certificate of Compliance shall contain:

2.1Contractor's name.

2.2Contractor's address.

2.3Contractor's phone number.

2.4Date.

2.5Contracting Agency's name.

2.6Contract Number.

2.7National Stock Number (NSN).

2.8Item nomenclature.

2.9A statement certifying that all items furnished on the contract are in full compliance with all specification and contract requirements.

2.10Authorized personnel's signature.

2.11Authorized personnel's printed name and title.
