Preparation Instructions
1Reference Documents.The applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as specified in the contract.
2Format.The FSI index shall be in a format similar to that of the sample form of Figure 1 of this DID.
3Content.The FSI index shall contain all of the information specified in the sample form of Figure 1 of this DID and as specified in MIL-STD-3034A, paragraph 5.1.3.3.
3.1Block 1. System/Subsystem ESWBS Number.Enter the highest level Expanded Ship Work Breakdown Structure (ESWBS) number to be covered by the index.
3.2Block 2. System/Subsystem Nomenclature.Enter the associated system/subsystem nomenclature for the ESWBS number specified in block 1.
3.3Block 3. Ship Class.Enter the ship class and hull number to which the RCM analysis applies.
3.4Block 4. Prepared By.Enter the analyst's name and the date.
3.5Block 5. Reviewed By.Enter the first level reviewer's name and the date.
3.6Block 6. Approved By.Reserved for the Maintenance Coordinating Activity (MCA) approval signature and date.
3.7Block 7. Revision.Identify the revision level of the FSI index and enter e.g. "ORIGINAL", "A", "B", "C", etc., sequentially as applicable and the date.
3.8Block 8. ESWBS Number.Enter the entry from each Functional Failure Analysis (FFA) Report form, block 1, and from each Additional Functionally Significant Item (AFSI) Selection Report form, block 1 that had a "YES" in block 15, as applicable.
3.9Block 9. Nomenclature.Enter the entry from each FFA form, block 2, and from each AFSI Selection form, block 2, as applicable.
3.10Block 10. Location.Enter the entry from block 9 of the AFSI Selection form for the item (equipment only).
3.11Block 11. Serial Number.Enter a four-segment serial number as follows:
3.11.1Segment 1 - Enter the developing organization abbreviation followed by a slant (/).
3.11.2Segment 2 - For developers, enter the development authorization number followed by a slant (/); for other development activities, assign a development number followed by a slant (/).
3.11.3Segment 3 - Enter the number 118, indicating the FSI Index form, followed by a slant (/).
3.11.4Segment 4 - Enter the ESWBS number from block 1.
