Preparation Instructions
1Reference Documents.The applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as specified in the contract.
2Format.The SLA report shall be in a format similar to that of the sample form of Figure 1 of this DID.
3Content.The SLA report shall contain all of the information specified in the sample form of Figure 1 of this DID and as specified in MIL-STD-3034A, paragraph 5.1.6.1.
3.1Block 1. ESWBS Number.Enter the Expanded Ship Work Breakdown Structure (ESWBS) number of the system under analysis as defined in Phase 1 on the Master Systems and Subsystems Index (MSSI), block 8.
3.2Block 2. Nomenclature.Enter the nomenclature of the system under analysis from block 9 of the MSSI.
3.3Block 3. Ship Class.Enter the entry from block 3 of the MSSI.
3.4Block 4. Prepared By.Enter the analyst's name and the date.
3.5Block 5. Reviewed By.Enter the first level reviewer's name and the date.
3.6Block 6. Approved By.Reserved for the Maintenance Coordinating Activity approval signature and the date.
3.7Block 7. Revision.Identify the revision level of the SLA report and enter e.g. "ORIGINAL", "A", "B", "C", etc., sequentially as applicable and the date.
3.8Block 8. Item and Task Description.Enter the nomenclature of each item and beneath that the description of each servicing and lubrication task pertinent to that item. Include MRC SYSCOM control numbers of existing tasks, where applicable.
3.9Block 9. Location.Enter the compartment number(s) where the task is performed.
3.10Block 10. Quantity.Enter the quantity of items upon which the task is performed that are part of the system under analysis.
3.11Block 11. Previous Periodicity.Enter the most recently used periodicity for this task on this and similar items. If this is a new item, enter periodicity from e.g., manufacturer, technical manual recommendation, and engineering judgment, as applicable.
3.12Block 12. Material Specification.Enter the specification and symbols of any material used; e.g., oil, grease, fluid, etc. MIL-HDBK-267A contains lubricant information.
3.13Block 13. Analysis Decision.Enter action taken by analysis, e.g., NC-no change, OM-omit, CM-change material, CP-change procedure; and enter the revised periodicity, if applicable.
3.14Block 14. Explanation.Enter rationale and justification for the analysis decisions in block 13, including an outline of revised procedures and specify new materials as applicable.
3.15Block 15. Serial Number.Enter a four-segment serial number as follows:
3.15.1Segment 1 - Enter the developing organization abbreviation followed by a slant (/).
3.15.2Segment 2 - For developers, enter the development authorization number followed by a slant (/); for other development activities, assign a development number followed by a slant (/).
3.15.3Segment 3 - Enter the number 121, indicating the Servicing and Lubrication Analysis followed by a slant (/).
3.15.4Segment 4 - Enter the ESWBS number from block 1.
