This Data Item Description (DID) contains the format, content, preparation instructions and intended use information for the data deliverable resulting from the work tasks described in 5.1 of MIL-STD-3034A.

Preparation Instructions
1Reference Documents.The applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as specified in the contract.
2Format.The Documentation Control Sheet shall be in a format similar to that of the sample form of Figure 1 of this DID.
3Content.The Documentation Control Sheet shall contain all of the information specified in the sample form of Figure 1 of this DID and as specified in MIL-STD-3034A, paragraph 5.1.
3.1Block 1 - Development Group.Enter the development group number and name.
3.2Block 2 - Ship/Class.Enter the class, ship hull number, as applicable, upon which the development is based.
3.3Block 3 - Developer.Enter the developing organization name.
3.4Block 4 - Contract Number.Enter the contract number under which this development was conducted, if applicable.
3.5Block 5 - Revision.Identify the revision level of the RCM Documentation Control Sheet and enter e.g., "ORIGINAL", "A", "B", "C", etc., sequentially as applicable.
3.6Block 6 - Date.Enter the date when the entire development was completed. If the development was revised, enter the revision date.
3.7Phase 1.Enter 114, the last three segments of the Master Systems and Subsystems Index (MSSI) serial number.
3.8Phase 2.Enter 116, the last three segments of the Functional Failure Analysis (FFA) serial number.
3.9Phase 3.Enter 117, the last three segments of the Additional Functionally Significant Item (AFSI) selection form serial number in the left column adjacent to the FFA serial number for the applicable system, subsystem, equipment containing the FSI candidate. If an FSI candidate was not selected as an FSI and not listed on the FSI index, enter "NSI" near the AFSI selection form serial number. Enter 118, the last three segments of the FSI index form serial number prepared for each subdivision, in the right column near the horizontal line between the FFA form and FSI selection form serial numbers. Draw connecting lines from the related FFA form to the AFSI selection form and FSI Index form serial numbers, as applicable.
3.10Phase 4.Enter 119, the last three segments of the Failure Modes and Effects Analysis (FMEA) form serial number, adjacent to the associated FSI Selection form serial number for the items. Draw connecting lines to the FMEA form serial number from the AFSI Selection form and FSI Index form serial numbers, as applicable. If none of the functional failures of an item were selected for further analysis, enter "TERMINATED", near the FMEA form serial number for the item.
3.11Phase 5.Enter 120, the last three segments of the Decision Logic Tree Analysis (LTA) form serial number, in the left column. Align with the associated FMEA form serial number. Draw connecting lines to the associated FMEA form serial number. If the decision logic tree analysis did not determine a requirement for any tasks related to the item, enter "NO TASKS" near the Decision Logic Tree Analysis form serial number. If safety related design change recommendations were submitted, enter 122, the last three segments of the serial number of such recommendations, in the right column, adjacent to but not on the same line as the Decision Logic Tree Analysis form serial number.
3.12Phase 6.Enter 121, the last three segments of the Servicing and Lubrication Analysis (SLA) form serial number. Horizontally align with the FSI Index form serial number for the associated system and subsystem. If no servicing and lubrication tasks were required, enter "NO TASKS" near the Servicing and Lubrication Analysis form serial number.
3.13Phase 7.Enter 123, the last three segments of the Maintenance Requirements Index (MRI) serial number. Horizontally align with the entry in the Phase 1 column for the system and subsystems.
3.14Phase 9.Enter 124, the last three segments of the Task Definition serial number adjacent to the RCM Decision Logic Tree Analysis serial number. If there are SLA tasks, align with the Servicing and Lubrication Analysis form serial number that determined the task was required. Draw connecting lines to the corresponding serial numbers. If no MRC was prepared for the task definition, enter "NO MRC" near the task definition form serial number.
3.15Phase 12.Enter in the left column the System Command (SYSCOM) Maintenance Requirement Card (MRC) control number of each MRC prepared during this phase. Align with the associated Task Definition form serial number and draw connecting lines. Enter the Maintenance Index Page (MIP) number for each group of MRCs in the right column. Draw connecting lines between the MIP numbers and associated SYSCOM MRC control numbers.
