The Planned Maintenance System (PMS) Maintenance Index Page (MIP) provides a concise index and cross-reference to all Maintenance Requirement Cards (MRC) within the MIP set.

This Data Item Description (DID) contains the format, content, preparation instructions and intended use information for the data product resulting from the work task described in NAVSEAINST 4790-8C, Section 2-4.4.b and MIL-STD-3034A, paragraph 5.1.12 and Appendix C.

This DID is related to DI-PSSS-80986BA, Reliability-Centered Maintenance (RCM) Maintenance Requirement Index (MRI); DI-PSSS-80991B, Planned Maintenance System (PMS) Maintenance Requirement Card (MRC).

This DID supersedes DI-SESS-80992A.

Preparation Instructions

1Reference Documents.The applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as specified in the contract.

2Format.The Planned Maintenance System (PMS) Maintenance Index Pages (MIP) shall be in a format similar to that of the sample form of Figure 1 of this DID.

3Content.The PMS MIPs shall contain all the information specified in the sample form of Figure 1 of this DID and as specified in NAVSEAINST 4790-8C, Section 2-4.4.b and MIL-STD-3034A, paragraph 5.1.12 and Appendix C.

3.1Block 1. SYSCOM MIP Control Number.This number is provided by the Maintenance Coordinating Activity (MCA).

3.2Block 2. Date.Leave date blank unless provided by the Maintenance Coordinating Activity (MCA).

3.3Block 3. Nomenclature - Ship System, System, Subsystem, or Equipment.Enter the ship system, system, subsystem, or equipment nomenclature and ESWBS number for the lowest level to which all the listed requirements apply.

3.4Block 4. Reference Publications.Enter the identifying number of the publication providing the most comprehensive maintenance information for the applicable system, subsystem, equipment for which the PMS documentation is being prepared, e.g., Operational Procedures (OPs), NAVSEA instruction books, manufacturers' pamphlets, NAVSEA technical manuals, etc. If additional space is required, enter the phrase: "See Note." A note shall be entered in the Maintenance Requirements (MR) Description block to provide the additional referenced publications.

3.5Block 5. Configuration.Maintenance Index Pages that index MRs for Expanded Ship Work Breakdown Structure (ESWBS) group levels 1, 2, or 3 or those that combine more than one ESWBS level 4 grouping, shall contain the level 4 ESWBS numbers and nomenclatures. The level 5 item numbers and nomenclature shall be included; this may include e.g., military designation, manufacturer name, Commercial and Government Entity (CAGE) code and part number. Level 5 items are typically subassemblies of the equipment under analysis, and normally designated as a sub-group of the equipment. When directed by the Maintenance Coordinating Activity, enter the change configuration of the applicable system, subsystem, equipment for which documentation is intended. Each SHIP CHANGE, FIELD CHANGE, ORDALT, and other officially authorized hardware change considered in the development shall be included. Each change that affects maintenance procedures shall be preceded by an asterisk; e.g., ORDALTS: *7167, 7380, *7662. When individual elements that make up the system or equipment being documented are not readily identifiable as part of the system or equipment, these elements and their individual change configuration shall be listed in this block. When other information regarding the composition of the applicable system, subsystem, equipment being documented will facilitate maintenance it will be included in this block. Use brief statements, e.g., Level of Repair Analysis (LORA) configuration or serial numbers 7-30 only. If additional space is required, the configuration data shall be continued in the MR Description block by use of a note. When the configuration data is so voluminous as to be impractical to include on the MIP, enter the identification of the publication from which configuration data can be obtained. In those cases where configuration data is not required, this block shall not be included on the MIP.

3.6Block 6. Scheduling Aids.All notations affecting the scheduling of MRs shall be located in the MR column preceding the first MR description. When scheduling aids include more than one topic, they shall be typed in boldface and numbered under a scheduling aid heading; e.g.:

3.6.1Scheduling aids:

3.6.1.1Schedule MRC A-3 for accomplishment during annual crypto reload.

3.6.1.2Review MRCs A-9 and A-11.Omit MRCs which do not apply; no feedback report required.

3.6.2Asterisks and the symbol # shall be used to call attention to specific scheduling information as follows:

3.6.2.3# Mandatory scheduling required.

3.7Block 7. Other.Enter the specialized management code, as applicable, listed below. If the MRC is also classified, a two-character code will be entered. The second character space shall identify the MRC's security classification. If there is no applicable specialized management code but the MRC is classified, the classification code shall be in this column. H - Denotes a Calibration Maintenance Requirement developed under the cognizance of the NAVSEA Calibration Program. K - Denotes a MR, inspection, etc., developed under the cognizance of PMS 392T, the Hull, Mechanical and Electrical Engineering Division. R - Denotes a MR on a Deep Submergence System (DSS) asset, as well as system and equipment that is within the Scope of Certification (SOC) where Re-entry Control (REC) documentation is required. RC - This MRC is confidential. Denotes a MR on a DSS asset, as well as system and equipment that is within the Scope of Certification where REC documentation is required. N - Denotes a MR on Moored Training Ship (MTS) systems and equipment that requires Moored Training Ship Support Yard (MTSSY) concurrence and NAVSEA approval. C - Denotes Confidential classification. S - Denotes Secret classification. X - Denotes an MR on a DSS asset, as well as system and equipment that is within the SOC where no REC documentation is required. XC - This MRC is confidential. Denotes an MR on a DSS asset as well as system and equipment that is with the SOC where no REC documentation is required.

3.8Block 8. SYSCOM MRC Control Number.Enter the control number of any reapplied MRCs as recorded on the MRC. In the case of newly developed MRCs, the control number will be applied by the Maintenance Coordinating Activity.

3.9Block 9. Maintenance Requirement Description.The MR description shall appear exactly as it appears on the applicable MRC. The first line of the first maintenance description shall be two spaces below the scheduling aid and column border if no scheduling aids apply. When the MRCs aggregated on a MIP are applicable to a number of systems, equipment, units lower than the one shown in the MIP heading, the specific system, subsystem, equipment unit shall be entered in boldface type (or underlined) on the line preceding the actual MR. Both item name and nomenclature designation shall be provided.

3.9.1Notes.Notes shall be used to define "R" requirements in the operational maintenance section of the MIP and to continue entries too numerous for the space provided in the Reference Publication and Configuration blocks. In addition, notes shall be used to provide management information applicable to a specific MRC; e.g., "NOTE: This MRC is applicable to Mod 1 only". The note(s) shall be placed immediately after the last MR description for that MRC and shall be unnumbered. If more than one note is required for the same MRC, the note defining the "R" requirement shall be listed first.

3.9.2Scheduling Notations.The first scheduling notation shall be located at least two spaces below the top line which borders the MR Description block. Two spaces shall be allowed between each scheduling entry.

3.10Block 10. Periodicity Code.Enter the periodicity code as shown on the MRCs. The MRCs shall be listed in the following sequence of decreasing frequency: D, 2D, 3D, W, 2W, M, 2M, Q, 4M, S, 8M, 9M, A, 18M, 24M, 30M, 36M, 48M, 54M, 60M, R, and U. Within each periodicity group, list the MRCs in numerical sequence, e.g.: D-1, D-2, D-3, W-1, W-2, M-1, M-2, Q1R, Q-2, A-1, 18M-1, R-1, R-2, U-1, U-2. When reapplying previously developed MRCs, the condition may occur where the existing MRC is applicable but the number associated with the periodicity falls out of consecutive order. Skipping of sequential code numbers is acceptable; e.g.: Q-1, Q-3, Q-4, Q-7. Asterisks are used in the periodicity column to call attention to scheduling information applicable to a specific MR. The standard use of the asterisk is listed below.

3.10.1Use a single asterisk (*) following the periodicity code indicates a Safety of Ship itemand requires the user to comply with COMFLTFORCOMINST 4790.3. The designation of Safety of Ship items will be recommended by Maintenance Development Activity (MDA) by annotation on the verification copy of MIP; for example, Q-1*. The Safety of Ship notation shall be entered on the verification copy.

3.10.2A single plus (+) following the periodicity code indicates that this procedure is intended to be scheduled and performed by Off-Ship Maintenance Personnel.

3.10.3A double plus (++) following the periodicity code indicates that this procedure is intended to be scheduled and performed by either Ship's Force or Off-Ship Maintenance Personnel.

3.11Block 11. Rates.Enter the rates required as shown on the MRCs.

3.12Block 12. Man-Hours.Enter the man-hours for each rate shown on the MRCs.

3.13Block 13. Related Maintenance.Enter the periodicity codes for MRCs which are identified as related during the procedure evaluation process. Entries in the related maintenance column shall be listed only with the MRC of the longest periodicity; e.g., if a weekly and a monthly MRC are related, only the monthly MRC would indicate the related maintenance. If two MRCs of the same periodicity are related, the related MRCs shall be entered with both MRCs. When the related MRC is from the same MIP, list only the periodicity code, e.g., M-3. When it is of another MIP, list the MIP series of the code also; e.g., "5600016: M-3". When the accomplishment of the related MRC is mandatory, # shall be placed after the periodicity code; e.g., "M-3#". When the related MRC is merely a convenience, the # shall be omitted. When there is no related maintenance, the word "None" shall be entered.

3.14Block 14. Unscheduled Maintenance.Entries for blocks Other, SYSCOM MRC Control Number, Periodicity, and Related Maintenance shall follow the convention set forth in their respective paragraphs above. These requirements shall be separated from the maintenance requirements by a horizontal line and use of boldface print. The MR description shall be written exactly as it appears on the applicable MRC. The first line of the first maintenance description shall be two spaces below the column border. A note shall be included for each unscheduled maintenance task to identify the situational requirement necessitating the unscheduled maintenance.

3.15Block 15. Inactive Equipment Maintenance.Entries for blocks Other, SYSCOM MRC Control Number, Periodicity, Rates, Man-Hours, and Related Maintenance shall follow the convention set forth in their respective paragraphs above. Enter SYSCOM control numbers from the applicable MRCs. In the case of newly developed IEM requirements, the SYSCOM control number will be applied by the Maintenance Coordinating Activity. The IEM requirements shall be listed on the MIP following the unscheduled maintenance requirements. The requirements shall be separated from the unscheduled requirements by a horizontal line and use of boldface print. The IEM section shall be titled Inactive Equipment Maintenance. The first entry following the title shall be: "The following requirements will be scheduled when equipment is inactivated for periods of prolonged idleness". When no IEM requirements are justified during IEM analysis, enter the words, "None required." The IEM requirements shall be entered as follows:

3.15.1The IEM requirements shall be grouped under the following subheads: Lay-Up Maintenance (LU), Periodic Maintenance (PM), Start-Up Maintenance (SU), and Operational Test (OT).The subheads shall be initial caps, underlined or boldface print.

3.15.2When Lay-Up Maintenance MRCs are prepared to support system, subsystem, equipment periods of prolonged idleness, Start-Up MRCs shall be required suitable for system, subsystem, equipment operation and activation.

3.15.3Within each subheading, IEM MRCs will either be a scheduling "Pointer MRC", a step-by-step procedure MRC, as well as a combination of both."Pointer MRCs" must reference a complete MRC. If only one MRC is referenced, the Maintenance Requirement (MR) description will duplicate the MR description on the MRC being referenced. If the "Pointer MRC" references a complete MRC with at least one MR, the MIP entry will be a generic MR description that encompasses all of the MRC tasks. If the "Pointer MRC" references multiple MRCs, the MIP entry will be a generic MR description that encompasses all of the MRCs tasks.

3.15.4The word "None" shall be entered for any subhead where IEM does not apply.

3.15.5Inactive Equipment Maintenance Periodicity Column.For new MRCs developed exclusively to satisfy IEM requirements, enter the periodicity code as shown on the IEM MRC. IEM "Pointer MRCs" will be given an IEM periodicity code applicable to the action being taken. The MRCs shall be listed in the following sequence: LU, PM, SU and OT.

3.15.6Inactive Equipment Maintenance Rates Column.MRCs with procedural steps shall include the rate(s) required to perform those steps. MRC pointer cards shall include only the highest rate that is required to perform an MRC listed.

3.15.7Inactive Equipment Maintenance Man-Hours Column.Inactive equipment MRs making reference to equipment operational MRCs shall contain 0.1 hr in the man-hours column. MRCs with procedural steps shall include the man-hours to perform those steps. If MRC is a blend of reference to operational MRCs and procedural steps, only include the man-hours to perform the procedural steps.

3.16Page Number.All pages of a MIP shall be identified by completing the entry "Page _ of _" on the bottom of the sheet.
