The Dosimeter Calibration Certificate will provide proof of dosimeter calibration, traceable to appropriate National Institute of Standards and Technology (NIST) traceable radiation standards. Each passive active (P/A) dosimeter shall be delivered by the vendor calibrated to monitor personnel to occupational radiation from photons, beta, and neutron.
This Data Item Description (DID) is applicable to contracts requiring National Voluntary Laboratory Accreditation Program (NVLAP) compliant calibration of passive active (P/A) dosimeters prior to delivery. All dosimeter calibrations shall be performed on the appropriate National Voluntary Laboratory Accreditation Program (NVLAP) phantom as described in American National Standards Institute/Health Physics Society (ANSI/HPS) N13.11 (2009); Section 3.4. Each dosimeter shall be delivered with a calibration certificate specific to the dosimeter by serial number.
This Data Item Description (DID) contains the format and content preparation instructions for the data product generated by the discrete Contract Line Item (CLIN) as delineated in the contract Statement of Work (SOW).
Preparation Instructions
2FormatThe Dosimeter Calibration Certificates shall be in the contractor's format.
3ContentThe Dosimeter Calibration Certificate shall contain the following content:
3.1Calibration certificatesCalibration certificates shall meet the requirements of International Organization for Standardization (ISO) 17025 (2017) Sections 7.8.2 and 7.8.4.
3.2Each calibration certificateEach calibration certificate shall contain, as a minimum, the following information:
3.2.1Title (e.g. "Calibration Certificate");
3.2.2The name and address of the calibration laboratory;
3.2.3The location of performance of the laboratory activities, including when performed at a customer facility or at sites away from the laboratory's permanent facilities, or in associated temporary or mobile facilities;
3.2.4Unique identification that all its components are recognized as a portion of a complete report and a clear identification of the end;
3.2.5The name and contact information of the customer;
3.2.6Identification of the method used;
3.2.7A description, unambiguous identification, and, when necessary, the condition of the item;
3.2.8The date of receipt of the test or calibration item(s), and the date of sampling, where this is critical to the validity and application of the results;
3.2.9The date(s) of performance of the laboratory activity;
3.2.10The date of issue of the report;
3.2.11Reference to the sampling plan and sampling method used by the laboratory or other bodies where these are relevant to the validity or application of the results;
3.2.12A statement to the effect that the results relate only to the items tested, calibrated or sampled;
3.2.13The results with, where appropriate, the units of measurement;
3.2.14Additions to, deviations, or exclusions from the method;
3.2.15Identification of the person(s) authorizing the report;
3.2.16Clear identification when results are from external providers.
3.2.17The measurement uncertainty of the measurement result presented in the same unit as that of the measurand or in a term relative to the measurand (e.g. percent);
3.2.18The conditions (e.g. environmental) under which the calibrations were made that have an influence on the measurement results;
3.2.19A statement identifying how the measurements are metrologically traceable;
3.2.20The results before and after any adjustment or repair, if available;
3.2.21Where relevant, a statement of conformity with requirements or specifications;
3.2.22Where appropriate, opinions and interpretations
3.3Each calibration certificateEach calibration certificate shall be delivered as a PDF file using the serial number of the dosimeter as the file name.
