The Engineering Change Proposal System Safety Report (ECPSSR) is used for each Engineering Change Proposal (ECP) or Change Notice (CN) to document the safety impacts over the system life cycle associated with the introduction of the proposed engineering change into the design. This may include additional supporting documentation such as analyses, test results, and trade-off studies.
a. This Data Item Description (DID) contains the content and format preparation instructions for the data product generated by the specific and discrete task requirement as delineated in the contract.

This DID is related to DI-SAFT-80101, System Safety Hazard Analysis Report (SSHAR); DI-SAFT-80102, Safety Assessment Report (SAR); DI-SAFT-80104, Waiver or Deviation System Safety Report (WDSSR); DI-SAFT-80105, System Safety Program Progress Report (SSPPR); DI-SAFT-80106, Health Hazard Assessment Report (HHAR); and DI-SAFT-82085, Hazard Tracking System (HTS) Data. Preparation Instructions
1Reference documents.The applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as specified in the contract.
2Format.The ECPSSR shall be in the contractor's format.
3Content.The ECPSSR shall include the following information resulting from a review of each ECP.
3.1The reason for the ECP.
3.2A list, in narrative or tabular form, of all hazardous areas associated with the ECP.Each hazardous area shall describe how the hazardous area would impact the system design.
3.3A list of existing hazards in the Hazard Tracking System (HTS) that would be impacted by the ECP.
3.3.1These impacts may be positive (e.g. a reduction of risk in the form of additional hazard controls) or negative (e.g. an increase of risk or reduction of the effectiveness of a hazard control).
3.3.2For each identified hazard, an estimate of how the hazard's MIL-STD-882E(C1) Table III Risk Assessment Code (RAC) could be affected by the ECP.
3.4If there are previously identified hazards associated with the ECP, these hazards shall be included in a tabular form that accounts for the same information as in the HTS to include the RAC.
3.4.1If it is determined that there are no hazards, a description of the analysis process and factors which allowed this conclusion shall be included.
3.5The safety impact of the ECP upon the existing system that summarizes all hazardous areas and impacted hazards.
3.6An identification of any update(s) of existing hazard analyses required to incorporate the effect(s) of the engineering change if the change is authorized.
3.7A listing of all of the software impacted by the ECP.
3.7.1For each unit of software identified, an estimation of potential impacts to the Software Control Category (SCC), Software Criticality Index (SwCI) and associated Level of Rigor (LOR) task activities.
3.7.2An identification of any update(s) to LOR task activities required to incorporate the effect(s) of the engineering change if the change is authorized.
