The Waiver or Deviation System Safety Report (WDSSR) is used to summarize results of analyses, tests and trade-off studies as they relate to a request for waiver/deviation. It shall identify the hazard risk assessment, mishap potential, and justification associated with results or each waiver or deviation request received throughout the system life cycle.
a. This Data Item Description (DID) contains the content and format preparation instructions for the data product generated by the specific and discrete task requirement as delineated in the contract.

Preparation Instructions
1Reference documents.The applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as specified in the contract.
2Format.The WDSSR shall be in the contractor's format.
3Content.The WDSSR shall include the following information resulting from a review of each waiver or deviation request.
3.1A statement of the specific safety requirement and its associated source document name and paragraph number, as applicable, for which a waiver or deviation is being requested.
3.2A detailed technical justification for the exception.
3.3Analyses to show that the mishap potential of the proposed alternate requirement, method or process, as compared to the specified requirement.
3.4A narrative assessment of the hazard risk involved in accepting the waiver or deviation.If it is determined that there are no hazards, the basis for such determination shall be provided.
3.5A narrative on possible ways of controlling hazards (e.g. reducing severity and probability) and existing compliance activities (if any).
3.6A narrative assessment of the Level of Rigor (LOR) task activity impacts involved in accepting the waiver or deviation.
3.7A narrative on possible ways of minimizing LOR task activity impacts
3.8Start and expiration date for waiver/deviation.
3.9Document the results of the task in the Hazard Tracking System (HTS) as appropriate.
