The Critical Safety Item, Characteristic and Critical Defect Report identifies components, assemblies and processes with one or more critical characteristics that if not conforming to the design data or quality requirements would result in an unsafe condition. This report also identifies the occurrence(s) of critical defects, event rates, investigation results, corrective action and notification actions.
a. This DID contains the format and content preparation instructions for the data product generated by the specific and discrete task requirements as delineated in the contract.
b. This DID supersedes DI-SAFT-80970.
c. This DID is applicable to the acquisition of designs or hardware with items considered to have, or potentially have, critical parts or characteristics in the technical data package.
d. This DID may be tailored on DD Form 1423 for the particular acquisition. If the acquisition includes both design and hardware, tailoring may not be necessary.
Preparation Instructions
1GeneralDefinitions applicable to this DID are as follows:
1.1Critical ItemA part, assembly, installation, or production process with one or more critical characteristics that, if not conforming to the design data or quality requirements, would result in an unsafe condition.
1.2Critical CharacteristicsAny feature (e.g., tolerance, finish, material composition, manufacturing, assembly, inspection process) of product, material, or process, which if nonconforming or missing could cause the failure or malfunction of the critical item.
2FormatThe Safety Report shall be in contractor format. Hard copy submittals shall be on 8 1/2 by 11-inch bond paper. Electronic/computer generated reports approved by the procuring activity shall be printable in the same format. When necessary to provide clarification (e.g., identification of the items, parts, assemblies, and materials); detailed diagrams, drawings, sketches, photographs, etc. shall be included in conjunction with the text. When consistent with an effective presentation, the contractor-selected format used in the initial submission shall be used for subsequent submissions.
3Content for DesignThe Safety Critical Items/Characteristics Report content shall be as follows:
3.1Title PageThe title page shall identify the contract number, the reporting period, and the date of the report.
3.2Table of ContentsThe table of contents shall identify the major topics addressed in the report.
3.3Critical Item Characteristics ListThe lists shall identify the following elements:
3.3.1Critical item identificationincluding the part number, nomenclature, and National Stock Number.
3.3.2Associated critical item characteristics
3.3.3The Commercial and Government Entity Code (CAGEC)that identifies the manufacturer of the critical item.
3.3.4Analytical data and resultswhich support the determination that an item and associated characteristics are critical.
3.3.5Documents which control the critical characteristics
3.3.6The service life, if established, for the critical item
3.3.7The most effective inspection method to be performed on the critical item to inspect for the critical characteristicsThe inspection method shall include any applicable test requirements (e.g., test equipment, test conditions, detailed test procedures, test results).
4Content for Report of Hardware Containing Critical Safety or Critical Defect
4.1NotificationThe notification information shall include:
4.1.1Prime Contractor name and contract number
4.1.2Subcontractor name(If defect occurred at a subcontractor).
4.1.3Item Identity(Part Nomenclature, Drawing Number and Revision).
4.1.4Specification Requirement(Specification, Paragraph, Characteristic).
4.1.5Lot Identity(Lot number and lot size).
4.1.6Defect Quantity this Occurrence(Total critical defects this contract).
4.1.7Cumulative Defect Quantity(Total critical defects this contract).
4.1.8Quantity of Suspect Material(Identify segregated lots and sizes).
4.2Detailed InvestigationInvestigation Information shall include: A narrative description of the detailed investigation and analysis of the critical defect. The narrative shall include the root cause of the defect, evidence that the segregated material is not affected and corrective action(s) taken to eliminate the defect in remaining material and to preclude recurrence.
4.3Segregated Material Use and Restart RequestThe request(s) shall include:
4.3.1Reference to the approved notification, detailed investigation and corrective action(s)
4.3.2Identify of material, lot number, and quantity
4.3.3Narrative request for use or restart authority
