The Operating, Support, Health, and Safety Hazard Analysis Report (OSH&S) Report are used to systematically identify and evaluate health hazards, evaluate proposed hazardous materials, and propose measures to eliminate or control these hazards through engineering design changes or protective measures to reduce the risk to an acceptable level.
This Data Item Description (DID) contains the format and content preparation instructions for the data product generated by the specific and discrete task requirements as delineated in the contract.
Preparation Instructions
1FormatThe OSH&S report shall be in the contractor's format.
2ContentThe report shall contain a title page specifying the following:
aTitle/identification of the system/component/program
dPreparing activity or contractor's title
eSecurity classification, when required
gDate of generated document
3ContentThe report shall contain the following sections:
3.1System descriptionThe system description shall contain a summary description of the physical and functional characteristics of the system and its components.
3.1.1System capabilities, limitations, and interdependence of these componentsshall contain relevance to system safety. Reword.
3.1.2The system and components (software included)shall be addressed in relation to its mission and its operational environment. System block diagrams and functional flow shall be included when applicable to clarify system descriptions.
3.2ReferencesThis section shall identify all source documents i.e., reports, standards, criteria, technical manuals and specifications including industry specifications and standards used.
3.3Hazard analysis resultsThis section shall contain:
bIdentified health, operating and support, and safety hazardsand shall include the following information:
(1)Identify the system/subsystem/unit that the analysis was performed for
(2)Components/subsystem failure mode(s)All failures modes which can result in a hazard. Failure modes generally answer the question of "how" it fails.
(3)System event(s) phaseThe configuration or phase of the mission the system is in when the hazard is encountered; for example, during maintenance, during pre-mobilization, installation, full-power applied, etc.
(4)System operation descriptionA description of what is normally expected to occur as the result of operating the component/subsystem or performing the operating/maintenance action.
(5)Hazard descriptionA complete description of hazard or hazardous materials. Also description of potential/actual hazard inherent in the item being analyzed or resulting from normal actions or equipment failure, or handling of hazardous materials.
(6)Hazard identification/indicationA description of operator/crew indications which include all means of identifying the hazard to operational/maintenance personnel.
(7)Effect of hazardThe detrimental effects which could be inflicted on the system, subsystem, equipment, facilities or personnel; resulting from this hazard. Possible upstream and downstream effects shall also be described.
(8)Risk assessmentA risk assessment for each hazard (classification of severity and probability of occurrence). This is the assessment of the risk prior to taking any action to eliminate or control the hazard.
(9)Recommendation action/mitigationThe recommended action/mitigation required to eliminate or control the hazard. Sufficient technical detail is required in order to permit the design engineers and the customer to adequately develop and assess design criteria resulting from the analysis. Include alternative designs and life cycle cost impacts where appropriate. Also include the effect of action/mitigation.
(10)RemarksAny information relating to the hazard not covered in other blocks; for example, previous failure data on similar systems or administrative directions.
