This report shall provide microwave tube failure information for tracking government furnished equipment (GFE) tube failures that have occurred during installation/integration phase of new production SPY Radar/Fire Control System (FCS) cabinets at the Contractor's facility.

This data item description (DID) contains the format and content preparation instructions for the data product generated by the specific and discrete task requirement for the data included in the contract.

This DID supersedes DI-MNTY-80175.

Preparation Instructions

2.0FormatThe Tube Failure Report shall be in the contractor's format.

2.1ContentThe report shall contain the following information:

aTitle of Report

bContract number and contractor's name

cGovernment microwave tube nomenclature and serial/Government Part number of failed tube

dDisposition of failed tube (e.g., warranty repair, warranty replacement, etc)

ePart number and serial number of cabinets and NHA

fDate of confirmed tube failure

gHull number of affected cabinet

hFailure Mode (General Description of Failure)including applicable specification or ATP paragraph number, contributing factors such as coolant or power loss, cabinet faults displayed at time of failure.
