This Data Item Description (DID) contains the format and content preparation instructions for the data resulting from the Reliability test requirements and failures described in the contract (see MIL-HDBK-781A for sample test methods).
Preparation Instructions
1Reference DocumentsThe applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as cited in the current issue of the ASSIST at the time of solicitation; or, for non ASSIST-listed documents, as stated herein.
2FormatThe report shall be in the contractor's format and shall be the same for both the Preliminary and the Final report.
3ContentThe Preliminary Report shall contain the following items:
The Final Report shall contain the items required in the Preliminary Report and the following additional items shall be included:
3.1Originator of the report
3.2Date of the failure/Incident (TIR block 40)
3.3Date of Hardware Receipt from Government
3.4Start date of Contractor's Report
3.6Failure Analysis Report Number
3.8Equipment, title, part number, and serial number
3.9Assembly title, part number, and serial number
3.10Subassembly title, element or module title, part number, and serial number
3.11Part name, part number, serial number, date code, and manufacturer
3.13Name and specification of test failed
3.14Elapsed time and phase of test failed
3.15Total operation time of unit at time of failure
3.16Test Phase Performed (i.e. vendor, CCA First Function, FAT, pre-range, in-water, post-range, etc.)
3.18Failure mode, Developer's Analysis of Problem (ref: TIR block 106)
3.19Classification failure (independent or dependent)
3.20Type of failure from Failure Keyword List (See 4)
3.21Operational and environmental conditions under which the failure occurred
