The Failed Item Analysis report is to be prepared for each failed item which is contractually required to be analyzed. It presents the results of failure analysis, methods of analysis, failure histories and corrective action taken or recommended.
This DID contains the format, content, and intended use information for the data product resulting from the work task described in the contract Contractual Engineering Task (CET).
Preparation Instructions
1Reference DocumentsThe applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as specified in the contract.
2FormatContractor's format is acceptable.
3ContentThe following shall be included in the Failed Item Analysis report:
3.1The Software Metrics Report shall include the following
3.1.3Name of engineer responsible for failure analysis
3.1.4Nomenclature and generic number of failed items
3.1.5Type of test during which failure occurred
3.1.6Operational and environmental conditions under which failure occurred
3.1.7Description of failure systems by which failure was detected
3.1.8Description of item's mode of failure
3.1.9Description of specific laboratory method employed in failure analysis (e.g., x-ray, spectrographic analysis, dissection)
3.1.10Description of failure cause
3.1.11Impact, if any, of environmental conditions on the failure cause
3.1.12Description of specific analytical method employed in identifying failure causes
3.1.13Failure history to date of subject item
3.1.14Recommendation for corrective action
3.1.15Failure analysis number
3.1.16Manufacturer of failed part
